We are currently seeking leaders to be the first to be trained in a new program, Powerful Tools for Caregivers, coming to Living Healthy Champlain. This training will take place March 22 & 23 at 43 Bruyere st. from 8:30 am-4:00 pm.

The training is FREE to attend.

Powerful Tools for Caregivers is an evidenced based program, developed over 3 years of pilot testing, based on the highly successful Chronic Disease Self-Management Program developed by Dr. Kate Lorig and her colleagues at Stanford University.

In 2007, PTC received the National Family Caregiver Award for innovation, responsiveness and effectiveness from the National Alliance for Caregiving and the MetLife Foundation. In 2009, PTC received the Network of Multicultural Aging Excellence Award from the American Society on Aging/AARP. In 2012, the PTC program was recognized by the Administration on Aging as having met its highest level criteria for evidenced-based programs.

The workshops are very similar in delivery to the Stanford CDSMP in that it is a scripted program, delivered over six weeks, and includes tools such as brain-storming, action planning and problem solving.

They apply and model self-management skills in their own lives. We hope you will consider becoming a volunteer group leader for this worthwhile program. You will have fun, meet new friends and help others to positively self manage.

How will the organization benefit?
Our leaders enable us to continue to offer our workshops. Leaders are individuals interested in teaching and empowering others to live a healthy life. As a leader, you will have the satisfying task of integrating the contents of the program and learning group facilitation skills that can be transferred to other areas of your life.

Thousands of patients and caregivers have been trained to lead Self-Management Workshops in over 25 countries in all kinds of communities.
